Salary : £52,000 to £62,000/year + NHS pension + CPD funding

About the Clinic :
This modern GP practice and walk-in clinic in South London serves over 12,000 patients and is part of a larger Primary Care Network. The multidisciplinary team includes GPs, physiotherapists, mental health practitioners, and social prescribers. They're now seeking an experienced Advanced Nurse Practitioner to play a lead clinical role in same-day access, chronic disease management, and patient triage.

Your Missions :
- Provide autonomous patient assessments, diagnosis, and prescribing
- Lead chronic disease clinics (diabetes, asthma, hypertension)
- Support urgent care presentations in collaboration with GPs
- Supervise junior nurses and HCAs
- Contribute to QOF and CQC compliance audits

Profile Required :
- Registered Nurse with current NMC registration
- Master's degree or PGCert in Advanced Clinical Practice
- Independent Prescriber (V300)
- Experience in primary or urgent care settings
